Shouko vanished because, with Sakuta still alive, she got Mai’s heart transplanted instead, which severed the inherent link between them. So while Shouko was still projecting her future self into the present, she became a different version of Shouko, whom Sakuta meets later on.

This is how I interpreted it.

After Mai dies, the Maki(nohara) with Sakuta's heart is replaced by the Maki with Mai's heart. Then Sakuta makes it so neither him or Mai dies, thus Maki dies, but she uses her adolescent syndrome to go back to the "present" which is the past for the people watching. And all the actions are more or less reset. So Maki goes back to elementary school, gets better, never had the adolescent syndrome thus never met Sakuta, thus they don't remember outside a "dream" the disappeared "future" turned into.

I heard they decide to continue the novel
but it seems it's still stuck in vol 11 and it's been a long time there's been no news
