In the manga version of Meakashi-hen, he seemed to have warmed up to her at the very least, before he disappeared (aka went L5 and was brought to Irie's clinic). Don't forget the way Satoshi reacted to Shion's kindness towards him, when he thought she was still Mion. Her actions flustered him, and when he did find out the difference, he pointed out how he noticed she had been acting differently than Mion normally would. He at least has a basic idea of when "Mion" was really Shion, so if he felt any certain feelings then, they'd be redirected towards Shion.
He also asked her how to write her name in kanji, and then after that he told her that "it's a good name". Which is a good sign!
We know Shion is waiting for him to wake up, after Matsuribayashi-hen. The whole time he's in the coma, Shion will be waiting for him, talking to him and reading books to him like she said. There's no reason to doubt her words there; we know how dedicated she is to him. He definitely heard her voice, too, because we saw him shed a tear as Shion was talking to Irie.
So, undoubtedly when he wakes up Shion will be at his side. He'll find out she's been taking care of Satoko for him the whole time, as well (as her "nee-nee"). I have a feeling that even if he didn't like her/know he liked her before, he'll end up liking her after all of that. <3
