Some of the characters in Space Brothers spend some time in Florida and Texas, which I guess could be considered foreign since most anime is set in Japan. I don't know if they do anymore exploring beyond that—aside from outer space, obviously. I'm only on episode 31. It's a nice series, but I'm taking my time with it.

I've only watched a little more than half of Michiko to Hatchin, but I'd say it's a pretty realistic portrayal of Brazil. The Portuguese is spot on, and its depictions of slums or favelas are pretty accurate. Not that I've been in a real life favela or anything. ;)

Oh and Maria-sama ga Miteru 3rd, last episode. They go to Italy and the visual surroundings are spot on.

Updated the OP with most of the shows you guys mentioned. For now, I'm just going to stick with shows that are set entirely outside of Japan, preferably where where the country is specified and emphasized throughout the series.

I'll start adding statistics once we have a few more titles.

How about the fact that only one show on the list represents South America and even then, it isn't explicitly? And nothing for Africa, period.
If anything, this just goes to show how preferential anime is towards Europeans.

And why wouldn't it be; It's not like Japan had historical ties with Africa.

Though there is an Africa arc in Nadia... Why don't you like the tag World Adventure?

For this lists I wanted to focus specifically on anime that accurately depicts life in one country (or in some cases, continent). I can add World Adventures, but my initial feeling was that an anime that hops around from place to place wouldn't have enough time to capture the culture and environment of each place it visits.
