Which adaptation is the most faithful?

The main Manga adaptation is quite faithful to the original LNs. Well based on the latest chapter at the moment they are already at Chapter 33 which is adapting Volume 6 of the Light novels. Although the English translation is still quite far though.

As for the TV series it is decent adaptation because the original author himself is the one editing on the screenplay on the series. Although he changes some bits to match or to take advantage of the medium.

Eriri - Manga at least at the start of the series, later on pretty much with the anime. As for Katou yes indeed shows some expressions on the LN when it counts, at most very subtle but her usual mode is her pokerface mode.
