Yes to first two and for your last:
According to NJAFC, The national anthem was played prior to short films. Ofuji was commissioned by the government to make an animation for it... This animation took him 3 tries till he was satisfied...

I wouldn't call it anime but it is a nice piece of animation, sort of like a music video, for the national anthem of Japan. It is animated with actual silhouettes, if you are interested.
